Super Super Big Doctor is the main antagonist of Phineas and Ferb the Movie: Candace Against the Universe.
At first, she seems to be a kind ruler, and even befriends Candace Flynn after saying she is the "Chosen One". But later, she is revealed to be a ruthless dictator and took over Feebla-Oot and enslaved everyone on it with her giant plant, Mama which needed Candace for carbon dioxide. After revealing to Candace that it produces mind control spores, Candace doesn't think that it's right so Super Super Big Doctor chains Candace to a treadmill so she would breathe out more carbon dioxide. Unfortunately, Candace lets it slip that all humans on Earth breathe out carbon dioxide, and so she sets an invasion for Earth.
When she and her crew reach Earth, a battle occurs between Phineas Flynn and the gang and her henchmen. When Mama grows big enough to grow mind-controlling spores, Candace uses some psychological tactics to make her realize that she never felt special because she was often overshadowed by her younger brothers. So she made herself special by becoming a tyrannical overlord. However, during her epiphany, she is eaten alive by Mama, who gains enough carbon dioxide to grow to gigantic size and achieve sentience.
Soon, Super Super Big Doctor and Mama are sent back to Feebla-Oot when Candace uses the Chicken-Replace-inator on both of them. When they return, Mama shrivels up and spits out Super Super Big Doctor, who is promptly placed under arrest by the Cowards for her crimes.

- Her name originated from Jim Bernstein, one of the writers, referring to his doctor as a "Super Super Big Doctor."[1]
- She represents a darker side of Candace's desires to have power over her brothers.
- Tiffany Haddish was initially cast in the role and even provided the temporary vocals for the animatic. However, she was recast with Ali Wong when Haddish was already playing a similar role in The Lego Movie 2: The Second Part. Haddish still appeared in the film, voicing "The Sound Someone Makes When They Explode from the Waist Up".
